Who is Nudj Health and why are they involved in my care?

Encora Health medical providers use Nudj Health as the staffing company that provides experienced behavioral health professionals, health coaches, registered dietitians, and exercise coaching staff.  These professionals deliver high-quality care using the Collaborative Care Model, Remote Patient Monitoring, and Chronic Care Management.  All three of these treatment models are research-backed treatments that extend Encora Health’s ability to treat patients in the comfort of their homes and get them feeling better and back to living a more vibrant life more quickly.  Encora Health providers are the ordering, supervising, and billing providers and allow us to coordinate care seamlessly, get faster patient results, and have a more effective integration of behavioral and medical healthcare services.

COLLABORATIVE CARE (CoCM)
What is Collaborative Care?

Collaborative Care uses a research-backed approach to healthcare to improve your health. Your care is delivered by a team of experts in many different areas and includes your ordering physician, your lifestyle medicine physician, care management staff, and a psychiatrist consultant. The care management staff may include nurses, clinical social workers, health coaches, registered dietitians, exercise physiologists, yoga instructors, and fitness specialists. Most chronic diseases develop slowly over time because of our lifestyle and behavior choices. Psychiatrists are experts in human behavior and emotions and help your care team support you in reaching your goals.

What are the lifestyle areas I can choose to work on?

Can I choose to focus on two lifestyle areas at the same time?

No. The program is designed to help you focus on one lifestyle at a time to give you the best chance to develop healthy habits that you will continue far after the program ends.  After graduating from one lifestyle area, you will be able to choose another lifestyle. You will experience the most health improvements when you focus on one area at a time.

How many appointments do I have per month?

Each month, you will meet with your specialist for a one-on-one appointment and attend two group classes for education and experiential learning. You will also have access to the Nudj Academy classes which you can take throughout the month to enhance your health goals.

How long is the program?

Each lifestyle area program is designed to take 16-weeks to complete when you follow the program prescription of 2 group classes and 1 one-on-one per month. To complete the program, you must complete all the classes and appointments.  If you miss one or more classes, you may take longer than the 16 weeks to finish the program.

What are the qualifications of the Nudj clinical team?

The team includes your ordering Encora physician or provider, the Nudj Health care management staff, and a psychiatrist consultant. The care management staff may include nurses, clinical social workers, health coaches, registered dietitians, 200 hr and 500 hr  registered yoga teachers, exercise physiologists and fitness specialists.

What is the cost of the program? 

Most insurance plans cover the Nudj program. Our Patient Navigator team will discuss your insurance coverage during your enrollment call. Important: If you have not yet met your deductible for the year, you will be responsible for the out-of-pocket cost until you meet the remaining amount. After meeting the deductible, you will only be responsible for the co-pay and/or co-insurance. If your co-pay and/or co-insurance is high, you can also ask about our direct pay program where you opt out of using your insurance. In some cases, this may be a less expensive option.

What should I do if I need to reschedule my appointment?

You can contact your health coach or care manager via email, text or phone call to reschedule. They will work with you to get you rescheduled for the current month to ensure that you follow the program prescription and complete the program in the allotted 16 weeks. Please note that you must cancel an appointment at least 24hrs in advance to avoid incurring the $25 no show fee. There is more information about this in the No Show Policy section.

What is the No Show Policy? 

Appointments must be canceled more than 24hrs in advance.  If canceled less than 24hrs in advance, a $25 fee will be charged.  Your appointment reminders and Welcome Email will include reminders of the no-show policy.  This policy is designed to help support your success in the program and to respect the time and efforts of our staff and other patients.  You can reschedule at any time or cancel within 24 hrs of your individual appointment or group class without being charged the fee.

Why does the Explanation Of Benefits (EOB) I received from my insurance company list Collaborative Care as a psychiatric service?

Your explanation of benefits (EOB) will list Collaborative Care as psychiatric services while you are in the wellness/risk reduction program. This does not mean you have been diagnosed with a psychiatric illness. Instead, it refers to the fact that the team of psychiatric consultants supervise and direct the lifestyle program behind the scenes by interacting with your care team. Psychiatrists are experts in behavior change. You have a team of experts working with you to guide you on your path to great health. We all need a village of support when making changes to our lifestyle. This collaborative team is your village!

What are the CPT codes billed for Collaborative Care?

Collaborative Care is billed to your health insurance with the following CPT codes: 

  • 99492: Initial Collaborative Care Management appointment

  • 99494: Additional 30 minutes for initial appointment

  • 99493: Subsequent Collaborative Care Management appointment.

  • 99494: Additional 30 minutes for subsequent Collaborative Care Management appointment

  • G2214: Less than 30 minutes – Initial or subsequent Collaborative Care Management

Who will bill my insurance?

Your healthcare provider will be the one billing the insurance and not Nudj Health.

Why did I not receive a weight scale?

Not all insurance plans cover remote patient monitoring (RPM) services, which is required to receive a weight scale. Please confirm with your Registered Dietitian to see if this is the case. If you would like to pay for the weight scale that will automatically upload measurements to the Nudj Health app, please discuss this with your Registered Dietitian.

How often should I weigh myself?

Nutrition patients: We ask patients in a nutrition coaching program to weigh themselves daily unless instructed otherwise by your Registered Dietitian because frequent tracking helps build awareness of progress, identify trends, and reinforce healthy habits. Daily weighing provides real-time feedback, allowing patients and coaches to make timely adjustments to nutrition and lifestyle choices. Research also shows that regular self-monitoring can improve accountability and long-term success in achieving health goals. However, we emphasize that weight is just one metric, and the focus remains on overall well-being and sustainable lifestyle changes rather than short-term fluctuations.

Heart Failure patients: Weigh yourself first thing in the morning after using the restroom and before you have breakfast or anything to drink.

Remote Patient Monitoring – Blood Pressure Program
What is the Remote Patient Monitoring Program?

Remote Patient Monitoring (RPM) involves using a connected electronic Blood Pressure or Weight scale device to record personal health and medical data. This helps providers monitor patients between clinic visits and can be used to monitor long-term and new conditions. The continuous tracking is helpful for patients with ongoing care needs, such as those with diabetes, heart conditions, asthma, and hypertension. Getting in the habit of checking blood pressure and weight daily can help you be more successful at managing your medical conditions and seeing what impact lifestyle choices can have on your overall health.

Will my Encora medical provider have access to my readings?

Yes, your Encora medical provider will have access to the Nudj portal via a computer or the Nudj Health app. Blood pressure and weight readings will be transmitted to the Nudj Health team for review. The Nudj RPM team reviews hundreds of these readings daily.  Your doctor has already given the Nudj Health team instructions on what measurements are too high or too low.  When the Nudj Health team sees one of these abnormal measurements, the team will follow your doctor’s instructions which can include calling you to check on you and/or notifying your doctor and his or her staff.

How often should I take my blood pressure?

We recommend it at least twice a day: once in the morning right after you wake up and using the restroom and once in the evening before you go to bed. Taking your blood pressure at the same time every day is helpful to identify trends.

How are the blood pressure readings transmitted?

Your device connects to AT&T cell towers. No Wi-Fi connection is required.  When the device is done transmitting your reading via cell towers, the device will automatically turn off. When it turns off this is evidence that your readings have been successfully transmitted.

How often are the blood pressure measurements reviewed, and who is reviewing them?

The RPM team reviews the blood pressure readings that get sent from your device within 24 hours of receipt on regular business days, which are Monday-Friday, except for some holidays. Transmissions sent on Saturday and Sunday and any holidays will be reviewed the following business day.

Does my provider have access to my blood pressure measurements?

Yes, your provider can access your Nudj Profile and view your readings on the internet or in the Nudj Health app.

What happens if I have critically high or low blood pressure?

If a measurement from your device is critically low or critically high, we inform your provider during business hours. If it's above 200> or <75, the Nudj team will follow the procedure listed above and contact you to ensure everything is okay. If your blood pressure readings are outside the the safe limits prescribed to you by your doctor, please follow the medical instruction provided.

How do I know Nudj is receiving my transmissions?

You can see all your transmissions on the Nudj Health app, if you can see your reading in the application, so can we. If you go two days or more without receiving a blood pressure reading, you will receive an automated message reminding you to take your blood pressure.

Can I travel with my blood pressure monitor?

Yes, you can travel with your device as long as it is in the U.S. or a U.S. territory. If you plan on traveling, please be sure to let us know so we can shut off your reminder notifications.

How do I set up my Blood Pressure and/or Weight Scale?

Only insert the batteries and place the cuff on your arm to take your first reading. The device will send the transmission straight to our platform. We also provide a Quick Start class to help you set up your device. This quick start class is a great way to learn the basics of the device, we can confirm receipt of your transmission and learn more about the RPM class offerings and programming available to you.

Which arm should I place the cuff on?

If this is your first time using the blood pressure cuff, please take the blood pressure reading on both arms first (unless you have a port in place, arteriovenous fistula, or lymphedema – in which case please only take it in the arm without the aforementioned). 

The arm with the highest reading will be the correct to use moving forward. IMPORTANT: Please notify your doctor on your next visit if your blood pressure was more than 10 points higher in one of the arms (note which arm had the 10+ reading).

What are the CPT codes bill for RPM?

Remote Patient Monitoring is billed to your health insurance with the following CPT codes:

  • 99453: Initial patient set-up.

  • 99454: Remote patient monitoring – Data transmission.

  • 99457: Time spent managing Blood Pressure transmission.

  • 99458: Additional time spent managing Blood Pressure transmission.

Why does the billing date not match my appointment date?

All billing codes for Remote Patient Monitoring are submitted at the end of a calendar month and not on the date of any appointments or the dates you took your blood pressure or measured your weight. Exception: CPT 99453 is the only code billed at the end of the billing cycle.

Who will bill my insurance?

Your provider will be the one billing your insurance and not Nudj Health.

Chronic Care Management
What is Chronic Care Management?

Chronic Care Management is a program designed to help you manage two or more chronic conditions. If you choose treatment with Chronic Care Management you are declining Lifestyle Medicine treatment with Collaborative Care. Chronic Care Management is a program with

  • A personalized and comprehensive care plan, shared and created with you, to help you manage your diseases and any needs impacting your health

  • Coordination with outside resources as appropriate

  • Assistance with medication compliance and symptom management

  • Monthly check-ins with your care manager

  • Remote monitoring of your blood pressure and/or weight, if prescribed by your referring doctor

Chronic Care Management is billed to your health insurance with the following CPT codes:

  • 99487: Service 60 minutes – Chronic Care Management

  • 99489: Additional 30 minutes – Chronic Care Management
